Transjugular Intrahepatic Portosystemic Shunt for the Management of Medically Refractory Chylous Ascites in Cirrhosis: A Single-Institution Retrospective Analysis

This retrospective analysis of only eight patients explored the use of a transjugular intrahepatic portosystemic shunt (TIPS) to treat stubborn chylous ascites (milky fluid buildup) in individuals with cirrhosis who hadn't responded to other medical treatments. The study found that TIPS led to resolution of ascites in most patients (7 out of 8), with a mean transplant-free survival of 19 months, but also noted significant complications including a 25% incidence of hepatic encephalopathy and two patient deaths.

Ketogenesis mitigates metabolic dysfunction-associated steatotic liver disease through mechanisms that extend beyond fat oxidation

In a small study of individuals with fatty liver disease, researchers found a positive correlation between the severity of liver damage and the rate of ketogenesis, a fat-burning process. Using mouse models, they showed that impaired ketogenesis reduced fat oxidation but did not always lead to more severe liver injury, hinting at a more complex role for ketogenesis beyond just burning fat.

Carbohydrate consumption and cirrhosis-related mortality: a prospective cohort study

This observational study found a link between higher carbohydrate intake, especially sugar, glucose, fructose, and sucrose, and an increased risk of death in patients with liver cirrhosis over a four-year follow-up period. Conversely, higher lactose intake was associated with lower mortality. It's important to note that this study cannot prove cause-and-effect.

Carbohydrate consumption and cirrhosis-related mortality: a prospective cohort study

In this prospective cohort study of 121 Iranian cirrhosis patients, higher intakes of total carbohydrates, sugar, glucose, fructose, and sucrose were associated with increased mortality risk over four years, while lactose intake was linked to lower mortality. However, the small sample size, single baseline dietary assessment, and observational design limit the ability to draw causal conclusions and generalize the findings to other populations. Further research with more robust methodology is needed to confirm these associations and explore the underlying mechanisms.
